Provincial Dementia Pathways Project
Collaborative Research to improve access to earlier diagnosis and ongoing care coordination for people living with dementia and their families in Alberta.
Alongside designing clinical pathways for earlier diagnosis and ongoing support, we are gathering evidence on how primary care and community services can help people with dementia and their families stay involved in their communities.
❋ Ethnographic WorkWe study in ‘real time’ how people adapt to a dementia diagnosis by having them share their everyday experiences with us.
We do this through ongoing visits them at home and in the community.
❋ Co-design of SupportsWe are meeting with groups of people living with dementia and care partners and clinicians to get their input on what works well and where there are gaps.
Talking with people across the province to build relationships and learn what is going on now - and listening to lessons they have learned about what works.
We want to partner with people and agencies interested in making life better for people living with dementia and their families.
